NDIS complex care and nursing in Cranbourne

Our NDIS complex care in Cranbourne brings registered nurses and trained support workers together for participants whose daily activities include complex clinical needs, including PEG feeding. High intensity daily support requires more than a roster. It calls for a current care plan, workers with the right training and a clear response when something changes. We take time to understand the person, the clinical instructions and the support already around them.

Where support happens

Depending on assessment, support can include complex personal activities and continence care. Continence assistance may cover toileting routines, products, skin care observations and record keeping within an agreed plan. Clinical decisions and procedures are guided by appropriately qualified professionals.

Before we begin

We review care documentation, identify competency and supervision needs, and agree who to contact for advice or escalation. Availability depends on the needs involved and whether we can safely staff the arrangement.

Getting started

Complex care at home: how we get started

Complex care starts with a clinical conversation. A registered nurse talks with you, your family and your health team about current care needs, existing care plans and any instructions from your treating professionals.

We then agree on how support will be delivered, which tasks need nursing oversight and how support workers will be trained and supervised for your plan. Complex care is reviewed regularly, and sooner if your health or needs change.

We keep clear records and, with your consent, share relevant updates with the people involved in your care so nothing important falls through the gaps.

Support is available in Cranbourne West and nearby suburbs, at home and in the community.

Funding and fit

NDIS funding for complex care and high intensity support

NDIS funding can be self-managed, plan-managed or NDIA-managed. Tell us how your relevant supports are managed and we will confirm the proposed service and payment arrangement before work begins.

We also agree the tasks, worker match, schedule, travel arrangements and relevant costs. Where clinical care is involved, assessment and staff capability are checked before the service is confirmed.
